The VHPA Disk shows #UH-1H # 66-17137 as being assigned to the 119th between Oct. 68 to Feb. 69. Looks like it ended service in Nam with the 190th AHC in Binh Hoa April 1970. It had a hard landing after engine failure resulting in split tail boom and spread skids, no injuries to crew. I will try to scan the report and put it on the page. If not I will email it to you.

Let me know if it's service dates with the 119th agree with your memory.
